Mississippi Action for Progress, Inc. ~ EHS Demonstration Center will be offering services to infants & toddlers – birth to three year old [on or before September 01, 2017] for the 2017 – 2018 school year.

There mission is to promote healthy outcomes for infants and toddlers and enable their parents to fulfill their roles as parents and assist them in moving toward self
sufficiency.

Services will be provided on a first come, first serve bases. These services are free of charge. Please provide the necessary documents to qualify your child.

Documentation Needed:
~Certified Birth Certificate
~Social Security Cards
~Immunization Record – Form 121 [Pediatrician or Local Health Department]
~Proof of Insurance
~Parents/Guardians Previous 12 Months of Income
~Information on Disability Status
~Documentation of Legal Guardianship – if not birth parent.
~Well Baby Visit Summary [Pediatrician Visits]
